About Permanent Supportive Housing:
Centre of Hope Permanent Supportive Housing (PSH) is a 17-bed program located on the 2nd floor of Centre of Hopes main building. This program works to provide an inclusive, harm-reduction and recovery-based home for individuals who were experiencing chronic homelessness prior to entering PSH and have been identified with extensive barriers to independent housing. Our PSH program is established from evidence-based practices to ensure each resident is provided with adequate support to increase their longevity in housing and self-efficacy. The program provides voluntary access to a flexible and comprehensive array of support services and places no limits on length of tenancy if terms and conditions of the lease or agreement are met. There is an extensive focus on resident engagement, behavioral health care, intensive case management services, life skills training, cultural components and more. The most integral components of the PSH program are resident choice, optional services, harm-reduction supports via an onsite managed alcohol program and protection of tenancy.
General Coordinator Duties:
Under supervision from the PSH Supervisor, the PSH Coordinator is responsible for but is not limited to:
· Providing support, advocacy and resource brokerage to individuals who were formerly experiencing chronic homelessness
· Assessing and addressing residents’ biopsychosocial needs via intensive and comprehensive case management service delivery
· Conducting regular assessments and associated interviews with residents to determine program eligibility
· Keeping comprehensive and detailed files and documentation in accordance with the provincial housing program standards
· Conducting and executing needs-assessment referrals followed with supporting residents to all necessary appointments, navigating the judicial system, and enhancement of life skills
· Providing crisis interventions and mediation between residents
· Assessing residents’ barriers to daily living and working to assist support staff with implementation of relevant life skills and behavioral plans
· Participate in conducting research to ensure the congregated PSH program is up to on all best practices within the field
· Working to address and close service delivery gaps for residents in relation to their medical, social, and behavioral needs
· Risk screening and safety plan development / implementation
· Case conferencing with community partners
Required Qualifications:
· Minimum 2 years working directly with individuals experiencing chronic homelessness (preferably within the Wood Buffalo Region)
· Bachelor of Social Work with active or eligible registration with the Alberta College of Social Workers (ACSW) or degree within a related field or equivalent combination of education and/or experience
· Must be able to navigate external support services and advocate on micro, mezzo, and macro levels
· Comprehensive understanding of indigenous ways of knowing, being and learning (preference to have knowledge on our regions outlying indigenous communities)
· Working knowledge of the Alberta Mental Health Act
· Comprehensive understanding of the Housing First model in accordance with recovery-based housing programs and the harm-reduction philosophy
· Advanced working knowledge of physical and social issues relating to activities of individuals who experience chronic homelessness
· Working knowledge of the common barrier’s individuals experiencing homelessness face
· Extensive knowledge of resources and services available within the community specific to homelessness
· Ability to work flexible hours which may include overtime, evenings, and weekends
· Valid standard First Aide and CPR level 5
· Class 5 drivers license and daily access to a reliable vehicle. Must be willing to transport residents
· Acceptable Criminal Record Check
Assets:
· Additional education or certification in addictions / mental health counselling or intervention strategies
· Certification in Motivational Interviewing, Narrative Therapy, Cognitive Behavior Therapy and/or related interventions
· Mental Health First Aid
· Service Prioritization Data Assessment Tool (SPDAT) training
· Applied Suicide Intervention Skills Training
