Morgan Place is seeking a part time (3 day/week) Social Worker.
Job summary: Responsible for the provision of social work services to a primarily geriatric population in both Long Term Care and Convalescent Care.
Qualifications: Graduation from an accredited school of Social Work and registration with the BC CSW. Experience working with a geriatric population in care as well as an overall knowledge of the health care system and community resources. Excellent communication and facilitation skills.
22.5 hours/ week
Duties to include:
- admission and discharge planning for both Long Term Care and Convalescent Care clients
- assessment and follow through with clients and their families as required
- participation in care conference, family meetings and convalescent care rounds
- liaise with members of the interdisciplinary team
- chair the Ethics Resource Committee and the Family Councils
- facilitate available community resources and contacts including assisting with financing and Public Guardian
- implement policy, procedures and routines
- participation in quality improvement initiatives
Job Types: Part-time, Permanent
Part-time hours: 22.5 per week
Salary: $39.00-$48.00 per hour
