POSITION SUMMARY:
The Occupational Therapist will promote and/or maintain functional independence of patients in the areas of selfcare, productivity and leisure in order to maximize patient quality of life in accordance with the code of ethics of the Professional College and Association and the Mission and Values of Unity Health Toronto.
MAJOR RESPONSIBILITIES:

Assess physical, psychological, perceptual, cognitive, vocational, and functional status through administration of formal and informal assessments for each referred patient;

Formulate and implement individualized client centered goals based on the analysis and assessment findings and consistent with team goals;

Monitor change and adjust therapy program as appropriate;

Provide adaptive equipment as required (i.e. wheelchair, seating orthotics, assistive devices etc.);

Assist with patient's discharge plans and pre-discharge preparations;

Establish individual patient's level of functioning within his/her home environment through home assessment. Provide recommendations to facilitate safety and maximize function within the patient's environment;

Identify and participate in clinical education in order to remain current in professional practice areas through readings, attendance at internal and external professional development events and through consultation with fellow Occupational Therapists;

Promote the Occupational Therapy profession both within Providence Healthcare and externally through publication, speaking, education and other professional organization;

Participate in the development of educational initiatives for patients, families, staff, volunteers, students and relevant community organization.
QUALIFICATIONS/SKILLS REQURIED:
- Bachelor of Science degree in Occupational Therapy from a recognized University or equivalent;

Membership in good standing with the College of Occupational Therapists' of Ontario (C.O.T.O.);

Membership in the Canadian Association of Occupational Therapists', (C.A.O.T.) and the Ontario
-
Society of Occupational Therapists' (O.S.O.T.) is encouraged;

Knowledge and experience in Occupational Therapy practice in a health care environment, preferably with the target population of the Program served;

Knowledge of Occupational Therapy assessment techniques, therapeutic skills, modalities and their applications to the patient population;

Ability to work cooperatively and collaboratively within an interdisciplinary team;
-
Excellent interpersonal and communications skills;
-
Demonstrated commitment to professional development;
-
Very good attendance record;
-
Good performance record.
