We are seeking a Support Worker to provide home care services to individuals with disabilities. The Support Worker will be responsible for assisting with meal preparation, assisting with systems, lifting, behavior management, social work, driving, direct support, and cleaning.
Duties:
- Assist with meal preparation and feeding
- Assist with systems such as medication management and medical equipment
- Lift and transfer individuals as needed
- Implement behavior management plans as directed
- Provide social work support to individuals and families
- Drive individuals to appointments and community activities
- Provide direct support to individuals with disabilities in their homes
- Clean and maintain a safe and healthy environment
Requirements:
- High school diploma or equivalent
- Valid driver's license and reliable transportation
- Ability to lift up to 50 pounds
- Ability to pass a background check and drug screening
- Experience working with individuals with disabilities preferred but not required
- Excellent communication skills
- Ability to work independently and as part of a team
