We are seeking a skilled and compassionate Primary Care Nurse Practitioner to join our team in providing primary care services. This position will require experience in a primary care and/or long-term care settings.
Responsibilities:
- Provide comprehensive primary care services to patients of all ages
- Conduct patient assessments and develop individualized care plans
- Monitor patient progress and adjust care plans as necessary
- Educate patients and families on disease prevention and health promotion
- Collaborate with interdisciplinary teams to ensure high-quality patient care
- Maintain accurate medical records and documentation
Qualifications:
- Active RN-NP license
- Master's degree in nurse practitioner
- Minimum of 2 years of experience in primary care or related field
- Strong communication and interpersonal skills
- Familiarity with family planning methods and procedures
