Based in Spruce Grove, WestView Primary Care Network is Alberta’s 2nd Primary Care Network (PCN). Working collaboratively with family physicians the PCN strengthens primary health care delivery in the communities of Spruce Grove, Stony Plain, Parkland County, and surrounding Indigenous Communities.
We are currently recruiting a Community Connector (1.0 FTE – 38.75 hours/week).
The Community Connector (Youth) supports youth ages 11-25 and their families accessing various levels of community and mental health support and services. The Community Connector (Youth) works directly with youth and service providers to navigate and coordinate referrals, address barriers to access and care, and facilitate youth and family engagement activities in the community.
This position contributes to the implementation of the mission and vision of the Tri-Region Integrated Youth Services (IYS) Initiative, which is to ensure that youth and young adults aged 11-25 have improved and integrated access to mental health, primary care, and social services within the communities of Spruce Grove, Stony Plain, local indigenous communities, and the greater Parkland County.
The successful candidate will have experience working with youth and young adults ages 11-25, a good knowledge of primary healthcare and mental health care in Alberta, experience working in multi-disciplinary teams, professionalism, excellent verbal and written communication skills, experience in advisory councils and social media platforms, and demonstrated project coordination skills. Preference will be given to those who have completed a degree or diploma in a related field of study (human/health services, education, social work) and who have experience with youth that may be dealing with past/present trauma, family violence, mental illness, substance use, homelessness, and/or intellectual disabilities. Must have own vehicle and valid driver’s license.
